REMINDERS:
1. This form needs to be filled out including parent signature before any Tylenol or cough drops will be given; and/or
2. Any prescription medications, inhalers or nebulizers need a parent AND Doctors signature, as well as original packaging.
Parents can send a picture of the form via email, or fax it, whatever works best for you.
